Award to a USA nonprofit community agency or health organization for programs that improve the health of traditionally underserved populations. The award is intended to recognize and support innovative healthcare solutions from an independent private-sector or public entity that operates separately from a hospital or healthcare system. For consideration, a program must successfully address a well-defined, unmet need, and will be additionally judged on innovation and creativity, outcomes, ease of replicability, impact of the award, and future vision.

The winning program receives a cash prize of $100,000 at Premier’s Breakthroughs Conference and Exhibition.
The awardee must be able to show results covering a full one-year span prior to January 2021.
