Grant Description
Grants to Delaware, Maryland, Pennsylvania, and Washington, DC nonprofit organizations, government agencies, and schools for programs to address health equity and improve the quality of healthcare for underserved populations. Applicants are required to submit a preliminary application prior to a full application. Funding is intended to advance health equity and improve access to quality healthcare for people experiencing disadvantages due to their race, ethnicity, socioeconomic status, and more.

In 2023, the Foundation launched Together for CHANGE (Changing Health Access for Next Generation Equity), a new signature program that aims to advance health equity and improve access to quality healthcare for people experiencing disadvantages due to their race, ethnicity, socioeconomic status and more.

Applying nonprofit organizations must meet the following criteria:

  • Organizations must provide culturally responsive services to help improve access to quality healthcare for people experiencing disadvantages due to their race, ethnicity, gender, gender identity, sexual orientation, age, income, socioeconomic status, ability, citizenship status, limited English proficiency, literacy, housing status, geographic location or any other barriers to accessing healthcare. This may include but is not limited to:
    • Providing access to care services and coordination of care
    • Mitigating barriers that impact access to care services
    • Improving cultural responsiveness of organizations

Funding priorities for 2023 include:

  • Organizations promoting screenings, early detection, treatment and/or continuity of care for cardiovascular, renal, respiratory or immunologic diseases and/or cancers.
  • Organizations establishing new partnerships and broader coalitions of community-based organizations or expanding existing partnerships with the intent of breaking down institutional silos and facilitating access to quality healthcare through inter-institutional collaboration.

Additional Eligibility Criteria
To qualify for a Foundation grant, an organization is required to be a U.S.-based, nonprofit organization with a 501(c) designation or a public school, government entity or municipal institution that is eligible to accept tax-deductible, charitable contributions. Organizations that are a 501(c)(3) and also have an IRS designation as a 509(a)(3) are ineligible for funding.

Ineligible
The Foundation does NOT fund applications that focus exclusively on the following:
- Capital investments and unsolicited capital campaigns
- Medical education and/or training (i.e., training necessary to obtain a degree or license) for healthcare professionals that is more than incidental and that is the primary focus of the grant’s activities
- Research or clinical trials
- Healthcare provider salaries
- Faith-based programs that are NOT open to the community-at-large

The Foundation does NOT support:
- Initiatives outside the United States or its territories
- Individuals
- For-profit organizations
- Endowments
- Journals or advertising
- Political causes, lobbying, fraternal or social organizations
- Religious organizations whose activities are not open to the general public
- Nonprofit organizations that discriminate on the basis of age, race, ethnicity, religion, national origin, gender identity, sexual orientation, marital status, military service, veteran status or disability

Pre-Proposal Conference
The Foundation will be hosting an informational webinar on May 11, 2023 at 1:00 p.m. – 2:00 p.m. ET to discuss the application and selection process and give interested applicants a chance to ask questions. Pre-Application Information
Interested organizations must complete and submit a brief online grant application to be considered for funding. Applications will be open for eligible US-based nonprofit organizations to apply on the Foundation website, beginning May 10, 2023 through 5:00 p.m. ET, May 31, 2023. Applications must be submitted by the deadline to be considered. Applicants are encouraged to read the application preview form prior to applying online to collect or prepare the necessary information in advance.

Organizations selected as finalists will be notified and invited to submit a full, formal application through the CyberGrants platform beginning June 28, 2023 through July 26 at 5:00 p.m. ET. A selection of finalists will receive grants based on funding availability and decision by the Foundation’s Board of Trustees.

Application Process and Key Dates:
- May 10, 2023 through May 31, 2023 at 5:00 p.m. ET.: Online grant applications are accepted.
- Late June 2023: Applicants are notified of their grant application status. Organizations selected as finalists receive invitations to submit a full application.
- June 28, 2023 - July 26 at 5:00 p.m. ET: Invited finalists submit full grant applications online.
- October 2023: 2023 Grant Awardees are notified. Finalists not selected for funding are notified.
- Fall 2023: 2023 Grant Awardees are publicly announced.
- November 1, 2023*: Funded organizations launch grant activities.
- October 31, 2024: First year grant activities conclude.

Organizations selected for funding will need to review and sign a Letter of Agreement prior to receiving grant funds.

Estimated Size of Grant
The Foundation will accept grant requests of $200,000 – $250,000.

Term of Contract
Recipient Grant Awardees may also be considered for renewed funding on an annual basis for two additional years, contingent upon meeting objectives, funding availability and approval by the Foundation’s Board of Trustees.
